Just had the same thing happen (offensive coordinator). Created a new coach under “User Teams”, assigned to the Packers, fired their offensive coordinator, switched back to my main team, hired the newly made available offensive coordinator, switched back to the Packers and retired.

On the staff mod tree, you HAVE to unlock the right side of the tree first. If you don't, it bugs out so you can't unlock both.
